                          Membership Application Instructions

PLEASE READ AND FOLLOW ALL DIRECTIONS OR YOUR APPLICATION WILL BE DECLINED!

1) ADD INFO@AYAQUEST.ORG AND SHAMANISTA@AYAQUEST.COM TO YOUR EMAIL CONTACTS.  The response to your application will come from Info@ayaquest.org and all other correspondence will come from Shamanista@ayaquest.com.

2) YOU MUST BE 21 YEARS OLD TO APPLY AND YOU MUST EMAIL A COPY OF YOUR DRIVER'S LICENSE  (SENSITIVE INFO BLACKED OUT) TO SHAMANISTA@AYAQUEST.COM FOR AGE VERIFICATION.

3) YOU CANNOT SUBMIT AN APPLICATION FOR ANOTHER PERSON!!!!  THE APPLICATION MUST BE FILLED OUT BY THE APPLICANT THEMSELVES.

4) READ AND ANSWER ALL QUESTIONS CAREFULLY.  ALL QUESTIONS REQUIRE A RESPONSE!
 
5) DO NOT abbreviate your answers or your diagnoses.

6) Make certain all of your answers are HONEST, ACCURATE AND COMPLETE.  This includes ALL PAST DIAGNOSES as well as current ones!  Failure to list any medical issues, diagnoses, medications, etc., will be be cause for rejection of application and/or immediate revocation of Membership.  Failure to disclose medical conditions CAN lead to serious injury up to and INCLUDING DEATH.

7) It is IMPERATIVE that we both have AND UNDERSTAND ALL of your answers.  If a question doesn't apply to you, mark with N/A, NOT with no or dashes.

8) REVIEW YOUR APPLICATION BEFORE you submit it!  This application is just as important as job application, loan application, etc., and needs to be treated so.

YOU CANNOT RESERVE A SEAT IN A CEREMONY OR REQUEST SACRAMENT FOR A HOME CEREMONY OR PURCHASE MEMBERSHIP UNTIL YOU HAVE RECEIVED YOUR ACCEPTANCE LETTER.
